Complete and submit the online registration form using the link below.
Print the form below, complete it, and submit it in one of the following ways.
  • Drop it in the offertory basket.
  • Give it to Father Tom.
  • Scan it and email it to shmichurch@gmail.com.
  • Mail it to the church office at P.O. Box 188, Hamilton, MO  64644.
